MCS-150 — Motor Carrier Identification Report

The MCS-150 form is a specialized document necessary for submitting or revising your USDOT number. FMCSA obligates you to complete this form and refresh your details every two years.

If your business has ceased operations, submitting the MCS-150 is required to inform FMCSA. You can't deactivate your US DOT number online or by phone.

Neglecting to submit your Biennial Update will lead to the deactivation of your USDOT number. Non-compliance could also result in civil fines of up to $1,000 per day ($10,000 maximum).

If you are no longer utilizing your US DOT number, you must submit a notification of business cessation to prevent any unauthorized usage of your US DOT number.
